Health Home Care Coordinator
Division:Comprehensive Care FTE1.0 Summary Core Responsibilities
* Conduct a comprehensive assessment with each Health Homes participant to create a Health Action Plan, identifying medical conditions, family and community support, short- and long-term goals, and barriers to success, using approved assessment tools.
* Assist the patient to obtain needed medical, social, financial, legal, transportation, interpretation, housing and home care services. Implement, coordinate and monitor service plan.
* Provide emotional support and utilize motivational interviewing and other techniques to encourage patient progress.
* Work with the patients interdisciplinary team to promote health and also to reduce hospitalizations, Emergency Department utilization, and to assure the patient receives recommended primary and specialty care and preventive measures.
* Establish solid working relationships with representatives from public and private agencies to facilitate patient entry into these systems and to coordinate service delivery.
* Maintain updated knowledge of entitlement programs, including DSHS, Medicaid and Social Security and their requirements.
* Demonstrate sensitivity to coordinate care for patients and families from a variety of ethnic, cultural, social and economic backgrounds and with varied medical and developmental needs.
* Document detailed, accurate, and timely case records through the ICHS electronic health record and in databases of contracted Health Homes Program lead entities. Maintain case records in accordance with health home policies/procedures, agency standards and regulatory requirements.
* Facilitate conferences among the patients internal and external resources as needed. Identify and communicate social, emotional, and patientfamily stressors to members of the care team as they relate to patients goals and plan of care.
* Facilitate program workflow and improvements to achieve program quality and performance aims.
Position Requirements **Education** - Bachelors Degree in a social service or health care field such as social work, behavioral sciences, psychology, chemical dependency counseling, nursing, physicians assistant training, health administration or related field. Masters Degree preferred.
**Experience -** At least two (2) years experience working with individuals with a chronic condition or disability, or working in an outpatient primary care setting. One (1) or more year(s) experience in a multicultural health care clinic and direct service experience with bilingual/bicultural patients preferred. Asian Pacific Islander language proficiency preferred.
**Other Requirement(s) -** Willingness to travel within the Seattle area for visits with patients in the community.
